Care4Today® Connect is not intended to substitute guidance from your healthcare professional. You should make your healthcare professional aware of any changes in your health status.
Care4Today® Connect delivers reminders to most smartphones. If you have an iPhone or an Android device, you can download the app for free from the App Store or Google Play. You will receive reminders via notifications from the app.
We may ask you to submit personal information in order for you to benefit from certain features (such as newsletter subscriptions, tips/pointers, or order processing) or to participate in a particular activity (such as sweepstakes or other promotions).
For the Optimization Study, the information reported in the app will be used to further optimize the app. Only information needed for the study will be collected. Appropriate technical and organizational measures to protect your privacy and prevent unauthorized access will be put in place.
When connecting to your healthcare professional, you are agreeing to the terms in the Disclosure of Health Information which allows your healthcare professional the ability to provide initial setup assistance and ongoing updates of medications, appointments, and activities. To keep updates that your healthcare professional makes in sync with your application, please be sure to open the app and review any updates. Always consult with your doctor about what medications to take, when to take your medications, and your health-related activities. You can choose to disconnect and stop sharing data at any time.
Keeping your information secure is important to us. We seek to use reasonable organizational, technical, and administrative measures designed to protect personal information under our control. Unfortunately, no data transmission over the Internet or data storage system can be guaranteed to be 100% secure. If you have reason to believe that your interaction with us is no longer secure (for example, if you feel that the security of any account you have with us has been compromised), please immediately notify us in accordance with the "Contact Us" section.
If you sign up for Care4Today® Connect, we may send you emails regarding application maintenance and product feedback. We may also ask if you want to consent to providing Personal Health Information to your healthcare professional.
Johnson & Johnson Health and Wellness Solutions, Inc., its affiliates, and agents may remove all personal identifiers that you and other users of the application have entered and use non-identifiable information without restriction, including but not limited to sharing such information with third parties for research or commercial purposes. The personal identifiers that will be removed are: name, date of birth, dates, telephone numbers, email address, device identifiers and serial numbers, URL and IP address numbers, and any other unique identifying number, characteristic or code.
Use of the Care4Today® app for this study may use your phone’s data plan. However, you can decide to use the app only on wi-fi by managing the app settings to prevent the app from using your data plan. You are solely responsible for any costs you may incur through use of your iPhone or Android mobile device.
