The Provider Connect Care Portal is paired with a connected patient app that allows for visibility to any patient self-reported information related to medications, appointments, survey tools, and routine activity trackers.

The Care4Today® Connect Care Portal is the web-based provider solution delivering insight regarding a patient’s journey managing a chronic condition.  

How it Works:

Once a patient requests a connection via a unique site code, your team can accept the patient request and begin to assign or review patient activities as needed.

iPACE-CVD Program

The iPACE-CVD Study Program will explore the impact of an innovative digital health technology, Care4Today® Connect, on clinical outcomes, disease management, and medication adherence for patients with coronary artery disease (CAD) and/or peripheral artery disease (PAD).

The first part of the program is focused on optimization and is a prospective, single arm, multi-center, non-interventional study. The study will be conducted using the Care4Today® CAD-PAD mobile app, and study data will be reported by patients directly in the mobile app.
